Home Test Kit Letter: Exposure to COVID

Dear Family,

You are receiving a COVID-19 home test kit because your child was exposed to COVID-19 while at school.

Here is how you should use your test kit:

· Please help your child take the first test on the day you receive the test kit.

· Please help your child take the second test on the fifth (5th) day after exposure.

· Your child may continue to attend school as long as your child receives negative test results and does not experience any symptoms of COVID-19.

· If your child has COVID-like symptoms or feels ill before the fifth (5th) day, please have your child isolate and take the second home test immediately.

· If your child tests positive on either home test, your child must isolate for 10 days following the first positive result. Please contact your child’s school immediately to inform them and for further guidance.

Child’s name and class number

The last day your child was at school

The date the symptoms started

The date the COVID test was taken

Home Test Kit Letter: Symptoms of COVID

Dear Families,

You are receiving a COVID-19 home test kit because your child was experiencing symptoms of COVID-19 while at school.

Here is how you should use your test kit:

· Please help your child take the first test immediately.

· Please help your child take the second test 24 hours later.

· Your child may not attend school until they receive two negative home tests taken within 24 hours and have been fever-free for 24 hours without the use of fever-reducing medication OR if your child receives a negative test result from a PCR COVD test.

· If your child tests positive on either home test, your child must isolate for 10 days following the first positive result. Please contact your child’s school immediately and for further guidance.

The date the symptoms started

The date the COVID test was taken
